Summer Camp Information and Required Forms
We are so excited to welcome your child(ren) to My Gym Summer Fun Days. At My Gym we strive to always provide a safe, nurturing, and of course, FUN environment for all children. Our teachers are passionate about fitness, learning, and helping children to build confidence in their own abilities as well as the courage to try new things. Our goal is and has always been to create an environment where children feel loved and supported so their self-esteem and confidence can flourish. The teachers of My Gym Bel Air cannot wait to welcome your child(ren) to My Gym Summer Fun Days!
Each camper will need to turn in the following prior to the first day of camp:
- Health History form
- MD Dept of Health Immunization Certificate
- Medication Administration Form (for emergency use medication only, we will not hold or administer regularly scheduled medications or OTC medication)

Each day of camp children will need to bring the following:
- Labeled water bottle
- Nut-free (Tree-nut free/Peanut free) snack.
- Complete change of clothes (accidents happen, even for older children – it’s better to be prepared 😀.
