My Gym @ Medfield Children's Center Registration Form
Please fill out this form to enroll in our My Gym Mobile program at Medfield Children's Center!
My Gym Mobile classes will be offered Monday and Tuesday afternoon starting September 16th. Each 30 minute class will be broken up by age with a maximum of 14 kids per class. Classes will begin at 3:30 pm and 4:00 pm. Your child's exact class time will depend on the total number and ages of children enrolled.
Tuition: $50 every 4 weeks per child.
Make-ups: If your child misses a class, you are permitted one (1) make-up per 4 week billing cycle to be schedule and taken at My Gym Medfield. Make-ups must be completed within two (3) weeks of the original class date.
Practice & Play: As an enrolled member, your child is welcome to attend any of our scheduled Practice & Play times at My Gym Medfield. Please check the website for our most up-to-date schedule. Practice & Play is a parent supervised activity. Un-enrolled siblings are welcome to attend as well for a drop-in rate of $5.
Sibling discounts will be applied automatically - 10% off 2nd child.

Payments will be made by automatic withdrawal.
By checking the box below, I acknowledge that I understand and agree to the following enrollment terms.
Tuition: I elect to have my recurring tuition, currently $40, automatically charged to my credit or debit card every four (4) weeks from the date of my first scheduled class. I authorize my bank/credit card company to make my recurring payment by the method designated below and to post it to my account.
Cancellation: This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. Your child will automatically remain enrolled with My Gym Mobile until an official Cancellation Request Form (CRF) has been completed and submitted in person or via email or until the end of the My Gym Mobile program at your child’s school. It is agreed that, when a CRF has been received, the member will be responsible for one final payment on the next scheduled date. This agreement will terminate four (4) weeks from that last scheduled payment. Children will continue to attend through that date
Release: In case of emergency when I cannot be reached, I authorize the staff of My Gym Mobile to obtain whatever medical treatment deemed necessary for the welfare of my child. I further understand and agree that I will be financially responsible for all charges and fees incurred in the rendering of said emergency treatment, regardless of whether or not medical insurance would cover such charges and fees. I also agree to receive promotional materials from our local My Gym, its parent company - My Gym Enterprises, and/or its affiliates. I hereby give my consent to my child’s participation in the activities of My Gym and herby absolve, release and hold harmless My Gym and all of its officers, directors, shareholders, agents, representatives, attorneys, owners, successors, assigns, and other affiliates from any and all liabilities for any injury or damages that my child may suffer in connection with the activities sponsored by My Gym Mobile or in which my child may participate.
