RECURRING PAYMENTS

I hereby authorize My Gym to charge my credit card every four (4) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification.

I understand that recurring payments include access to unlimited classes. Some classes may be excluded in this promotion. 

Members must inform the gym of an absence in advance.  In the event of an absent or gym closure you will be able to book your child into another class through our unlimited class promotion. There will be no refunds or store credit for missed classes. 

I understand that unlimited classes must be booked in an age-appropriate class and some classes may not be included in the unlimited classes promotion.

I understand that classes not included in the unlimited promotion do not have make-up options and that I am assuming the cost of a missed class.

CANCELLATIONS

This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. Your child will automatically remain enrolled at My Gym until you cancel your enrollment and receive an email confirmation from the gym stating your cancellation request has been processed. The preferred method to request a cancellation is to do so from your My Gym account. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership. Non-attendance does not constitute cancellation. Refunds or credits cannot be issued for any missed classes.

 TERM

Class Tuitions are nonrefundable and nontransferable.
