Birthday Questionnaire

Thank you for choosing My Gym Potomac for your child's birthday party.

We are so excited to make it memorable and special. Please answer the questions below.

Your questions will help us create a unique party program for the star of the day.

Reminders -
- Adults wear socks on the gym floor, and children participate barefoot. We sell socks for adults for $1 a pair.
- Children 3 years of age and younger must be accompanied by an adult.
- All families must sign a safety waiver in order to participate.
- Please confirm all pizza and juice orders with us ASAP if you have not already. We don't allow pizza from outside vendors so please arrange pizza with us before your party.
- We can also provide Baskin Robbins Ice Cream cakes and juice if needed. Please pre-order with us ASAP.
- Arrive no earlier than 10 minutes before the party. Our staff will be busy preparing for your event. Balloons for decoration are included. We recommend keeping other decorations to a minimum.
- Gifts should be opened at home to maximize the children’s fun in the gym.
- Please plan to leave the gym no later than 10 minutes after the party so we can prepare for the next celebration.

If you are serving pizza we ask that you order through us. Let us know how many pizzas you would like to order and toppings. Large cheese pizzas are double cut into 16 slices. Cost is $16 each and $2 per topping.

Paper goods - Let us know if you are bringing your own paper goods or if you would like us to provide. Basic paper goods are included in the Deluxe package and themed paper goods are included in the Premier package.

Juice - Let us know if you are bringing your own juice/drinks for children or if you would like to order through us. Juice for the children is included in the Deluxe and Premier packages.
