Vaccination/Negative Test Proof Submission

Proof of full COVID-19 vaccination is required for everyone 14 years old and up to enter the gym for any duration of time. Those who can not show proof of full vaccination can show a negative result conducted no more than two days prior to their visit. A COVID-19 self-test, sometimes referred to as a “home test”, is not acceptable. You may display proof on a smartphone or by showing a physical copy of the proof, along with a government-issued ID. Guests younger than 18 may present a school photo ID.

You will receive a text message notification when your vaccine proof has been checked and approved. This may not happen immediately, we appreciate your patience. Please make sure the phone number you have provided can accept text messages.

Birthday party guests: if you have not RSVP'ed/Signed a waiver for your child for the party, please get in touch with the party family and ask for their unique RSVP link. This form is not the waiver to play, this is for vaccination/test submission only. (Waiver to play RSVP required for ages 0 years - 13 years. Vaccination/test submission required for ages 14 years old and up)
